Interview Sheets for Becoming Who I Want to Be
In an "assignment" in the latter part of the workbook, clients are asked to "interview" others who are close to them to find out how they think the clients are progressing in their work to attain their good life goals. The clients are told that their counselors can supply blank Interview sheets.
Instructions for completing an interview sheet appear on page 120 of the workbook. A sample of a completed interview sheet appears on pages 121-122. The blank sheet appears on pages 123-124. It can be torn out of the workbook, or photocopied out of the workbook, but downloading this pdf file will make the task of copying it a lot easier.
Note that this assignment will be more manageable for clients if the sheet is printed as a two-sided document. Each client will need several copies, as they are to use each sheet for one interview with one person.
